New Patients

How to register as a patient

If you wish to register as a patient at this practice please collect registration forms from the reception desk at the surgery.

When you return the completed Registration Form,  you will be given a telephone appointment with the Health Care Assistant for your New Patient Medical. If you take repeat medication please bring in a list of all your current repeat medication provided by your current surgery. 

All patients' are now registered with the practice rather than an individual GP. This means that when you attend the surgery to see a GP you can see a GP of your choice.

 We accept patients living in the following areas:

Tonypandy;  Clydach Vale; Llwynypia;  Ystrad;  Gelli;  Trealaw;  Dinas; Penygraig & Williamstown

Please do everything you can to keep your appointments. If you are unable to attend please contact us as soon as possible so that your appointments can be allocated to other sick patients.

Please let us know if you change your name, address or telephone number so that we can keep our records up to date.

 We ask that you treat the doctors and practice staff with courtesy and respect at all times.

 Violent and abusive behaviour

The NHS operates a zero tolerance policy towards those individuals who are verbally or physically abusive towards any member of the practice team.

Such behaviour may result in you being asked to leave the surgery and if necessary the Police may be called.

In these circumstances you may be removed from the practice list.

 St Andrew's Surgery provides a family focused primary care service. Our service will endeavour to maintain the highest standards of care, access and accountability to all patients.

We will endeavour to work in partnership with other health and social agencies and our community as a whole to improve and develop our services for the practice and our locality as a whole.
